Description & Requirement

The UNC Department of Family Medicine seeks an Advanced Practice Provider (APP) to be a primary clinical service provider in its REACH Home Care program. The Nurse Practitioner or Physician Assistant will join a team of attending physicians and RN care coordinators in providing regular in-home patient visits, including assessments, health maintenance visits, and regular monitoring of chronic conditions. These visits supplement, but do not replace the patients' relationships with existing primary care providers (PCPs).
The APP is an integral member of the care team, working collaboratively with Family Medicine attending physicians, care coordinators, and community PCPs to provide high quality, cost effective and seamless care to patients in home and domiciliary care settings. The APP is expected to perform duties and responsibilities with a significant amount of autonomy and self-direction. The APP will provide direct care including diagnosis, medication management, ordering of diagnostic and therapeutic procedures, performing in-home procedures within scope of practice, meeting with the care team, and ensuring timely and accurate documentation of all services. The successful candidate will have excellent communication and customer service skills, as well as proven excellence in clinical care.
Preference given to APPs with experience in providing home-based primary and/or palliative care.


EITHER Requires graduation from a Physician Assistant program approved by the NC Board of Medical Examiners and "approved to practice medical acts" based on education and experience by the Board of Medical Examiners, and preferably one year of experience as an extender; OR licensed as a Registered Nurse by the Board of Nursing and BSN and graduation from a Nurse Practitioner Program approved by the NC Joint Subcommittee of the Board of Nursing and Board of Medical Examiners, and "approved to practice medical acts" based on education and experience by the Joint Subcommittee, and preferably one year of experience as an nurse practitioner; or an equivalent combination of education and experience.
