Responsibilities may include:
•Performing comprehensive patient assessments.
•Obtaining physician orders for certifications and re-certifications of home occupational therapy services.
•Utilizing our electronic medical record system, McKesson, to complete documentation including OASIS.
•Developing appropriate home exercise programs.
•Delivering occupational therapy modalities appropriate to the patient's clinical status and needs.
•Performing transfer and discharge planning functions.
Requirements for this position include:
•Graduate of an accredited occupational therapy program.
•Occupational Therapy license.
•Two years of Occupational Therapy experience is preferred.
•Experience within the Home Health field is preferred.
•Valid driver's license, car insurance and reliable transportation.
•Willingness to travel throughout assigned service area.
